Complete Therapeutic Phlebotomy Checklist.

If the patient’s hematocrit is below 48% for females/52% for males or if the hemoglobin for a females is less than 16.5g/dl or 18.5 g/dl for males the physician must be notified before proceeding with the phlebotomy.  A physician may indicate on the patient’s order if they approve other criteria for performing the phlebotomy. Example: a lower hemoglobin or hematocrit level may be specified.
